Abstract

Pronator syndrome-median nerve entrapment at te elbow level is considered as a rare condition and it is more common among women among women than in men. Long-term retrospective 8.5 years follow-up study evaluating the outcome of surgical release of the median nerve at the elbow comprising 8 female machine milkers as never been carried out before. Furthermore as long-term follow-up study of 14 non-surgical treated female milkers with pronaor syndrome never been studes. In the present follow-up study two groups of machine milkers (surgical and non-surgical) were compared to each other. The clinical examination was focused on two parameters namely focal tenderness and individual muscle strength. The result showed that in the surgical group no one had any focal tenderness on palpation over the median nerve at the elbow in the 8.5 year follow-up study. With respect to the muscle to the muscle strength testing revealed any selective weakness in any of the muscles examined compared to which was found before the surgical release. Three milkers were still active in the follow-up. In the non-surgical groupter were seven active active milkers and the focal tenderness was found in 12 out of 14 milkers. Regarding the weakness 10 out of 14 women still showed the same weakness as 10 years before. Seven milkers were still active. While this study has litimitations in the sample size, surgical release of the median nerve at the elbow level will in case of a definite pronator syndrome, appears to provide an immediate as well as long-standing return to normal strength of FPL and FDP II along with a significant improvement of status subjectively. In the non-surgical group, spontaneous improvement of the strength of FPL and FDP II was only found in only four out of 14 cases
