Macrocytic hypochromic RBC are not always reticulocytes

Uppsala Sweden A Wachtelhund bitch had pale mucous membranes and previous laboratory testing had shown it to have low hematocrit, RBC count and haemoglobin concentration. On the day of the test, there was mild to moderate anemia which was very macrocytic and hypochromic [hematocrit 0.32 l/l (0.39-0.55), total RBC count 3.13 x 1012/l (reference interval 5.7-9,1), hemoglobin 86 g/l (138-204), MCV 103 fl (60-77), MCHC 264 g/l (335-365), reticulocyte percentage 28.6 %, absolute reticulocyte count x 897 109/l]. The percentage of macrocytic hypochromic RBC was 97 %. The total WBC count was 16.5 x 109/l (6-15). The platelet count was 368 x 109/l (170-490). This case illustrates that macrocytocytic hypochromic RBC are not always reticulocytes. There were 97 % macrocytic hypochromic RBC but only 29 % reticulocytes. The reason for this will be revealed at the case session at the meeting. The diagnosis was phosphofructokinase deficiency in the Wachtelhund
